You will report to the Manager, Care Integration Division
Conduct assessments of patients’ needs using the appropriate assessment tool.
Collaborate with health care professionals to ensure patients are placed at the most appropriate care settings
Provide care assessment expertise to AIC for advisory, training and audit purposes, and be involved in the appeal process
Requirements:
Minimum 3 years' experience in acute and / or community settings in Singapore
Knowledge in geriatrics and community services will be an advantage
Strong assessment acumen including an understanding of the health care system
Competent in the use and interpretation of InterRAI assessment tools
Proficient in more than 1 language and conversant in local dialects
Proficient in MS Office applications. Statistical knowledge will be an advantage.
Exhibit high degree of flexibility, adaptability, assertiveness, maturity and confidence
Must be a critical thinker, motivated and resourceful team player
Excellent interpersonal and communication skills
